Age: The shelter staff think I am about 4 years old. STAFF OBSERVATIONS 08/22/26 11:11 dog appears depressed in kennel - would not interact with handler despite coaxing - behavior may be due to medical - see medical notes for ore details. MEDICAL NOTES 08/19/26 13:00 -- General Attitude: BAR, Friendly, Easy to handle -- Eyes: Normal OS, OD conjunctiva appears swollen -- Ears: Normal, Scabbed over lacerations and abrasions on pinnas Nose: Normal Throat: Normal Mouth/Teeth: Normal -- Skin: Normal, Lacerations and abrasions all over head and neck, Worse wound is on the right side of neck, Worse is on top of forehead Large laceration with open skin flap with necrotic tissue -- External Parasites: Live ticks seen Haircoat: Normal Nails: Normal Legs: Normal Gait: Normal Tail: Normal -- Body: Normal except for wounds on head and neck Body Condition (1to 9): 5/9 Altered: Intact Breathing: Normal Coughing/Sneezing/Nasal discharge: None Vomiting/Diarrhea: None Pain score(CSU pain scale): 2.5/4 -- Major concerns summary: Large laceration on forehead with necrotic tissue, and another laceration on right side of neck, Some smaller lacerations and abrasions on neck face and ear pinnas Intake care performed: Weight: Done Photo: Needed Scanned for Microchip: None found, Implanted mchip -242 from lot 01747354 Intake vaccines: Done Rabies vaccine: Done Lot #: e273781a Expiration: 29oct27 Flea control/tick: Done Other care Provided: Gave 0.47cc IV Tel from bottle 26-465, 0.35cc Dex IV, 1cc IV Morphine from bottle tp25-340 @ around 9:56am. Clipped and cleaned all wounds with dil betadine. Gave 2.15cc SQ convenia, Gave 1.90cc SQ carprofen. DVM F debrided wounds and sutured them Starting on: Caprofen 50mg PO BID x 5 days, Gaba 300mg PO BID x 10 days, Simb 0.48cc IM BID x 5 days 08/19/26 10:55Pt presents with large 7-8cm laceration over forehead, there was a large flap of tissue hanging over the area and a 3cm laceration over the flap, another 3-4cm laceration was identified on the right lateral cheek, pt was sedated and the wounds were debrided with a scapel blade, there was a large portion of necrotic tissue in the corner of the flap, elected to close the wounds with the necrotic tissue to act as a graft of sorts, will likely need revision/further debridement, if wound fails to heal, may need tie over bandage and heal by second intention, some conjunctivitis also present, otherwise normal physical exam, no neurologic deficits noted, rx convenia, cerenia and carprofen inj, starting on oral gabapentin and carprofen, recheck wound daily 08/20/26 09:11 food seems to have been touched depressed necrotic tissue with slight discharge on top of the head mild swelling on top of the head sutures intact on top of the head Rx)Enrofloxacin 136mg ; 1 tab PO SID for 14 days plan)- monitor daily for abscess developing- consider debriding to expose the entire lesion and manage it by secondary intention healing using tie-over bandage 08/21/26 12:10 patient is a go slow but is handleable, patient was sedated with ttdex prior dvm approval, 0.31cc ttdex was provided IM (26-470) (16-15). vitals during sedation hr: 65, rr: 40. mm: pink, crt: <2, temp: 99.8. patient was waken up mid procedure and second half dose of ttdex was provided 0.15cc IM. patient could not be provided o2 or anesthia support due to vacuum exhaust being unavailable, patient was provided sq fluids, 800ml/hr. once sx procedure was done, patient was reversed with antisedan 0.15cc IM. patient appeared to have prolonged recovery and dvm was provided antisedan 0.07cc IV per dvm, waited for about 10 min and patient was slowly being responsive but dvm provided flumazenil iv. patient appeared to have rough recovery, hr rate ranging from 40-60, iv catheter 18g was placed on lt front cephalic. patient was placed on maintenance rate 100ml/hr. patient seems more aware, swallowing, moving head. per dvm patient might be possibly sensitive to telazol 08/21/26 09:15 food seems almost untouched overnight purulent discharge draining from the closed wound on forehead Tx)sedation debridement : removed all the sutures and necrotic tissue as much as possible. freshened up the wound margin with metz scissors and blade closed dorsal and medial skin flap using 3-0 PGCL. ventral side of the defect was left open tie-over bandage placement : 4 x tie over bandage loop was placed using 0 nylon honey dressing with gauze - tied down with umbilical tape enrofloxacin injection 22.7mg/ml : 6ml IM E collar Plan) tie-over bandage change in 2 days and then, every 3-4 days until the wound is healed or consider delayed primary closure if granulation tissue forms with enough skin Use torb+dex for bandage change--> P has prolonged recovery from TTdex 08/22/26 09:05 didn't eat or take oral meds overnight Depressed, sternal recumbency Tx)SQF LRS 400ml Cerenia 10mg/ml : 2.0ml SQ Plan)contact rescue - deadline tomorrow 8/23 by 3PM
