Grade 4 tartar, worn teeth, gingivitis Heart-grade 4/6 systolic murmur with synchronous femoral pulses. HR 80 Lungs-no ausculted abnormalities. RR 30 Derm-coat is dry M/S-WNL Neuro-WNL Urogenital-WNL Assessment: intact, adult male with ocular changes that suggest possible KCS; heart murmur and dental disease. Dog is in need of a full work up that includes a cardiac ultrasound. With a history of coughing, the dog may have advancing cardiac disease and be in need of a work up urgently. Plan: STT. If dog has KCS, will need additional diagnostics and treatment not available in the shelter. Dog is not available for foster or PUFA and will not be getting surgery at MCACC. Placing dog on portal, urgent medical NHO. If no outlet by COB on 09/01/26, is at risk for humane euthanasia on 09/02/26 or sooner if QOL is declining.
