Nose clean and free of discharge. External ears are free of discharge/redness. Oral - Unable to perform full exam due to sensitivity. Mandibular canines extruding and appear to have grade 4/4 periodontal disease. Appears to be missing some dentition. H/L/MM - Grade III/VI left-sided regurgitant murmur. Mildly tachypneic with increased expiratory effort and wheezing. No coughing or sneezing. No stertor, no stridor. Mucous membranes pink. GI - Abdomen soft, full palpation not performed. UROGEN - No testicles palpable. INTEG - Coat is clean and clear of redness and ectoparasites. NEURO - Normal mentation. M/S - Stiff hindlimb gait but readily ambulates when placed on floor. Sensitive to ROM of hindlimbs, unable to fully assess. L/N - N/E A: - heart murmur: r/o secondary to mitral regurgitation (given signalment) vs other underlying structural cardiac disease - wheezing: r/o secondary to pulmonary disease (i.e. pulmonary edema seconday to CHF, pnuemonia, pleural disease, etc) vs tracheal disease (i.e. tracheal collapse) vs other cardiac disease vs other - stiff hindlimb gait: r/o secondary to osteoarthritis vs spondylosis vs old musculoskeletal injury vs other - dental disease P: - updated to \"N\" in Chameleon - chest radiographs - add wet food to diet - not medically okay to PUFA at this time 8/29/2026 08/29/26 13:58 Only one lateral thoracic radiograph obtained due to patient's respiratory concerns and needing to avoid excessive restraint. A: - cardiomegaly: r/o secondary to mitral regurgitation vs other - intrathoracic tracheal collapse - rocks in stomach P: - patient needs outlet with NHP for further cardiac and respiratory work-up as well as dental cleaning/extractions - Dog is not suitable to PUFA/foster.
