Evaluation Comments (Observations from our staff and volunteers) 9/28/2026 Standing with a relaxed body. Allowed all petting/handling & took treats gently while being examined by veterinarian. Tossed treats into kennel for dog to fully enter. Allowed leash removal to be done by hand. *Interested parties should be made aware of all medical notes. Interested parties should be counseled on providing this dog with proper decompression time in a new home.  Allowing time to adjust is important for their comfort and well-being.  Adopters should be counseled on proper dog-to-dog introductions, which should only occur after the dog has fully decompressed. Based on kennel presence & intake notes, dog is behaviorally adoptable @ intake color. 9/27/2026 neutral body, pulled from kennel to hold for tech to examine, allowed all handling, sniffing handler, more aloof than soliciting attention, walked to yard to relieve, stop and go on leash, walks slowly, urinated in yard, soliciting attention, placing head and muzzle on handlers leg, allowing for all petting, soliciting petting from handler, walked back to ramp, muzzle wrap for vaccines, stood still for vaccines, no difficulty vaccinating, will pull away or stop walking when canine doesn't want to go somewhere or do something however overall soft, social, and calm disposition at time of intake, no difficulty handling, will resist entering kennel, curious of barking dogs, will perk up / ears up, enjoys hot dogs and PB 9/26/2026 friendly, able to load with muzzel wrap with out issue into truck kennel Medical Treatments 9/29/2026 09/29/26 14:57 Follow up exam done. Mass on ventrum is approximately 5-6 centimeters in diameter with an ulcerated ventral surface. On further examination, the poplitieal lymph nodes are significantly enlarged, irregularly shaped and firm to palpation with the right side > the left. Inguinal, prescapular and submandibular lymph nodes all palpate within normal limits. DDX (enlarged lymph nodes): regional infection, neoplasia, open. The prognosis is guarded to poor. Dog is now No Surgery. Dog is not available for PUFA or foster. Is in urgent need of a work up to determine the cause of the enlarged lymph nodes. Follow up with family vet. Place on the portal, urgent medical NHO. If no outlet by COB on 10/06/26, dog is at risk for humane euthanasia on 10/07/26 or sooner is quality of life declines. Continue current therapy as prescribed. 9/28/2026 09/28/26 10:18 P is BAR; BCS: 4.5/9; age updated to ~7 years EENT: OU clear with no ocular discharge or blepharospasm. AU cropped with mild-moderate debris Dental: Grade 2/4 Cardio/Resp: HR/RR WNL; no murmurs auscultated. Normal bronchovesicular sounds bilaterally; eupneic GI: Great appetite; no stool observed in kennel. Abdomen soft, nonpainful on palpation Derm: Ulcerated, mildly pendulous dermal mass along left ventral chest near second mammary/nipple region. Thick calluses/pressure sores present over bilateral elbows and lateral hocks. L hock callus with scabbing, superficial ulceration; thickened/scarred skin bilaterally Neuro: Normal mentation Musc/Skel: Ambulatory x4 with no ataxia or lameness observed Urogenital: Intact male with 2 descended testes A: - Ulcerated dermal mass L ventral chest; r/o: benign cutaneous mass vs cyst vs neoplasia - Chronic pressure calluses/sores bilateral elbows and hocks with superficial ulceration L hock; r/o: pressure-related dermatitis vs secondary bacterial infection - Aural debris AU - Dental disease grade 2/4 P: - Rx: Cephalexin 250mg - Give 3 capsules PO BID x 14 days - Clean ears AU at time of neuter surgery - If time and staffing allow, remove L ventral mammary/thorax mass at time of neuter surgery - Provide soft bedding to reduce pressure on callused areas - Medically okay to PUFA as long as adopter is aware of full medical record and the need to follow up with family veterinarian for further evaluation 9/27/2026 Nobivac 3-Rabies, manufactured by Zoetis, Serial number 859498: administered SQ, R lateral hip, by _ADR#713__, supervising veterinarian ST.
