Placing Green due to kennel presence and intake notes. 8/29/2026 Walked to triage room, allowed handling and petting. Vaccinated with light restraint. Walked to kennel. Medical Treatments 8/30/2026 08/30/26 14:20 P is BAR, friendly on exam; BCS 3.5–4/9 EENT: OD cornea appears dry with moderate mucoid ocular discharge and crusting; OS with cataract - no blepharospasm OU. P appears visually impaired and hearing impaired Dental: Grade 3/4 Cardio/Resp: Grade 4/6 heart murmur, louder on right. Normal bronchovesicular lung sounds bilaterally; eupneic GI: Soft, non-painful on abdominal palpation; P has great appetite for canned food, fair appetite for kibble; readily eats hot dog treats; no stool in kennel Derm: Hair coat clean; two perianal masses adjacent to anus: one larger, smooth, raised, alopecic pink/tan mass (left side) and one smaller, lobulated pink mass with focal ulceration (right side) Neuro: Normal mentation Musc/Skel: Ambulatory x4 with no ataxia or lameness observed Urogenital: Intact male with two descended testes A: - OD dry-appearing cornea with moderate mucoid ocular discharge/crusting; r/o KCS vs conjunctivitis vs other; Cataract OS - Visual impairment - Mild hearing impairment - Heart murmur, grade 4/6, louder on right - Underweight, BCS 3.5–4/9 - Dental disease, grade 3/4 - Multiple perianal masses; r/o perianal adenoma vs adenocarcinoma vs other neoplasia; focal ulceration of smaller mass P: - Perform STT OU - Rx: BNP ophthalmic ointment – Apply OD BID x 14 days - Rx: Gabapentin 100mg - Give 2 capsules PO BID x 10 days - Feed 2x daily and add canned food to meals - Recheck weight in 14 days - Medically not okay to PUFA. No surgery at MCACC. P has multiple medical issues and needs follow-up with a family veterinarian for further cardiac evaluation, ophthalmic evaluation/treatment of ocular disease and visual impairment, full dental evaluation/cleaning/extractions, etc, and FNA/cytology of both masses; biopsy/histopathology - Dog is at risk of humane euthanasia on 9/9/26 if no outlet is found by COB on 9/8/26, but may be humanely euthanized earlier if QOL declines 8/29/2026 08/29/26 15:06 M Testicles palpable with adult reproductive features, consistent with unaltered male.
